Agency Funding Eligibility Criteria
All prospective applicants must meet Eligibility Requirements in order to apply for funding.
Applicants must be a 501(c)(3) or public, tax-exempt health and human service organization with at least a two-year history of providing health and human service programs and have program results that clearly align with UWHC goals in the areas of Education, Financial Stability, Health, or Basic Needs/Crisis Services. Agencies may combine resources and apply together in a new collaborative program addressing one or more of the HC2025 Target Issues. Agency overhead expenses must be below 25% of total expenses.
Documents required ONE TIME (when agencies apply for the first time)
• 501 ©3 letter
• Mission Statement
Documents required EACH YEAR
• Annual Financial Audit/Independant Financial Review (every three years)
• IRS 990
Other items that agencies must be prepared to produce if requested
• Summary of Insurance Policies
• Certificate of Accreditation (if they are accredited by a state or national body)
• Financial Policies and Procedures
• Articles of Incorporation
• By-Laws
• Code of Ethics
• Non-discrimination Policy
